You can download an adoption profile form for a cat, dog, or small animal (rabbit, guinea pig, hamster, mouse, rat). You can fax the completed adoption prfofile form to the PHS/SPCA customer service at 650/685-0102; mail it to us at 12 Airport Boulevard, San Mateo, CA 94401; or bring it to the shelter during our adoption hours: Monday-Friday, 11 am to 7 pm; Saturday and Sunday, 11 am to 6 pm.

If you are interested in adopting another kind of animal, please call adoptions at 650/340-8200 extension 700, or come to PHS/SPCA to talk to an customer service rep. Monday-Friday, 11 am to 7 pm;
Saturday and Sunday, 11 am to 6 pm.

Note that it is often not possible for our staff to complete adoptions for visitors arriving less than an hour before our posted closing times.
